MODULAR COURSE ON THE DIAGNOSIS AND TREATMENT OF SNORING AND SLEEP APNEA: STEP BY STEP

In recent years, sleep disorders in general, and obstructive sleep apnea and hypopnea syndrome (OSAHS) in particular, have become increasingly important. OSAHS has contributed decisively to a better understanding in society of a disease that occurs during sleep but has consequences throughout the day.
OSAHS is a public health problem that affects men, women and children. It causes increased cardiovascular risk, metabolic diseases, hypertension, traffic accidents and accidents at work.
Established treatments for this condition can be more or less invasive. There are several options with good results, and these treatments should be known to health professionals who intend to work in the field of sleep.
It is thought that only 10% of individuals with the condition are correctly diagnosed and treated. OSAHS is thus a serious public health problem, operating in obscurity.
Therefore, this course aims to train health professionals so that they can improve their assessment, diagnostic and therapeutic skills in obstructive sleep pathology in a multidisciplinary and interdisciplinary approach to the patient.

PROGRAM

Module I

Section I - Introduction to Sleep Medicine

  • The nature and functions of sleep;
  • Neurobiology and neurophysiology of sleep;
  • Classification of Sleep Disorders.
 

Section II - Patient Assessment

  • The patient interview and assessment;
  • Differential diagnosis;
  • The multidisciplinary organization of the Sleep Centre: Neurology, Pulmonology, Psychiatry, Psychology, Chronobiology, Physiotherapy, Speech Therapy, CMF, Physical Exercise, Nutrition;
  • Diagnostic exams and tests: Polysomnography and Polycardiography: an introduction to the devices;
  • Practical demonstration.
 

Section III - Sleep Disordered Breathing

  • Relationship between Sleep and Respiratory Disorders;
  • Pathophysiology of Obstructive Sleep Apnea;
  • Long-term consequences of Obstructive Sleep Apnea;
  • Clinical Approach to the Diagnosis of Obstructive Sleep Apnea;
  • Upper Airway Imaging in Obstructive Sleep Apnea;
  • Overview of Obstructive Sleep Apnea Treatment.
 

Practical Class (4h)

  • Clinical History and Physical Examination in Snoring and Sleep Apnea;
  • Patient assessment scales, questionnaires and quotation techniques/protocols;
  • Complementary Diagnostic Examinations - Observation and Discussion;
  • Polysomnography / Respiratory Polycardiography - technique of use, protocol;
  • Practical demonstration.

Module II

Section IV - Night Bruxism and Movement Disorders

  • Definitions, Epidemiology and Etiology of Night Bruxism;
  • Orofacial Movement Disorders during Sleep;
  • Clinical Approach to the Diagnosis of Night Bruxism;
  • Pathophysiology of nocturnal bruxism;
  • Night Bruxism in Children;
  • Interceptive orthodontics and OSAHS in children;
  • Control of nocturnal bruxism;
  • Comorbidities.
 

Section V - Sleep and Orofacial Pain

  • Pathophysiological Conceptualizations of Chronic Pain;
  • Mechanisms of Sleep Loss and Interaction with Pain;
  • Clinical Implications of Sleep Loss and Interaction with Pain;
  • Association between Orofacial Pain and Sleep Disorders;
  • Impact of Common Temporomandibular Dysfunction Comorbidities on Sleep Quality and Orofacial Pain.
 

Section VI - Oral Devices

  • Oral Devices: BTI Apnia System;
  • Oral devices: types, preparation techniques, protocols;
  • Guidelines for the manufacture of oral devices;
  • Dentofacial Orthopedics in the Treatment of Apnea and Snoring: Clinical Cases;
  • Physiotherapy for patients with snoring and obstructive sleep apnea;
  • Physiotherapy intervention in the preparation of patients who are candidates for oral devices and CPAP;
  • Adverse reactions to oral devices: Temporomandibular and other dysfunctions;
  • Approach protocol and therapeutic exercises.
 

Practical Class (4h)

  • Oral devices: types, working models, preparation techniques. Practical explanation;
  • Inter-occlusal records for making Oral Devices;
  • Practical evaluation of different types of devices.

Module III

Section VII - OSAS in children
  • Particularities of snoring and OSAS in children;
  • Pathophysiological considerations;
  • Consequences of OSAS in children;
  • The diagnosis of OSAS in children;
  • The Therapeutic Approach;
  • The Pediatrician's View.
 
Section VIII - The Otorhinolaryngology and Orthodontics Approach
  • Anatomical and Pathophysiological Considerations OSAS-ORL;
  • Medical and surgical therapeutic approach to otorhinolaryngology in children and adults;
  • When to intervene surgically in adults and children?
  • When to send to the ENT?
  • Orthodontic assessment of patients with suspected snoring and OSAS;
  • Early orthodontic treatment in children;
  • The advantage of certain orthodontic techniques such as rapid maxillary expansion;
  • Orthodontic-Surgical Treatment;
 
Practical Class (4h)
  • The Clinical History of OSAS in Children;
  • Questionnaires for children;
  • Physical examination in children;
  • The dentist's therapeutic approach to children with OSAS;
  • Review of Clinical History, Physical Examination and Questionnaires in Adults;
  • Review of oral devices, occlusal records and fitting techniques;
  • Review of clinical cases, diagnosis, complementary tests and treatment plan;
  • Clinical cases for clinical reasoning.

Material needed for the course to be brought by the student: 

Basic material: lab coat, goggles, pencil, pen, wax knife;

1 pair of study models of your own mouth (upper and lower) without a base, in type III or IV plaster, cut out so that they can be articulated manually.

Minimum number of trainees for the course: 10

Failure to attend one or more modules does not entitle you to a refund or a final diploma.
